Texas Consumer Rights Notice

(Required under Texas Health & Safety Code §181.105 — H.B. 4224, 89th Legislature)

1. How to Request Your Counseling Records

As a client, you have the right to access and obtain copies of your health care records. To request your records:

  • Submit a written request to Adam Ortman at adam.ortman@gmail.com. While verbal requests may be accepted, a written, signed, and dated request is recommended.

  • Specify the records you need (e.g., "all session notes from January 2024 through present" or "my intake assessment only").

  • Include identifying information — your full legal name, date of birth, and contact information so your identity can be verified.

  • Fees: A reasonable, cost-based fee may be charged for mailing records. Records will not be withheld solely because of an outstanding balance for services.

  • Response time: Your request will be addressed within 30 days.

2. Contact the Licensing & Disciplinary Authority

If you have questions about professional standards, want to verify my license, or wish to report a concern about professional conduct, you may contact:

3. File a Consumer Complaint

If you have a concern about billing practices, deceptive practices, or other consumer protection issues, you may file a complaint with the Texas Office of the Attorney General:
