1. How to Request Your Counseling Records
As a client, you have the right to access and obtain copies of your health care records. To request your records:

Submit a written request to Adam Ortman at adam.ortman@gmail.com. While verbal requests may be accepted, a written, signed, and dated request is recommended.

Specify the records you need (e.g., "all session notes from January 2024 through present" or "my intake assessment only").

Include identifying information — your full legal name, date of birth, and contact information so your identity can be verified.

Fees: A reasonable, cost-based fee may be charged for mailing records. Records will not be withheld solely because of an outstanding balance for services.

Response time: Your request will be addressed within 30 days.
